Due to the Privacy Laws, a signed release of information form from the NEW Medical Centre you are now attending is needed.
There is a cost involved in this process to cover for Photocopying & Posting. Once we have received the signed form an invoice for the costs will be created and posted to you.
When we have received payment, allow 10 working days for the process of releasing your medical history to the new medical centre.
Unless payment is received NO INFORMATION WILL BE RELEASED.
Depending on size of Medical History & request from your NEW Medical Centre on the format they want to receive it, there will be times when we will load the information onto a CD.
Costs for sending on CD will be $20 for the CD & $5 for Registered Post.
Costs for Photocopying will be .20c per A4 page + postage.
Patients without a valid Medicare card will be billed privately.
The current fee for a Standard Consultation (on weekdays) is: $100.00
Some consultations are Non-Medicare service items, and are not claimable through Medicare.

Some procedures may attract an additional fee (we’ll confirm this with you beforehand).
WorkCover consultations and medical assessments are billed in line with our standard arrangements.
