Please make an appointment with one of our caring nurses to complete a comprehensive health assessment to ensure all your medical needs are being managed. Once completed your doctor will review and discuss your assessment with you.

 

Do you have diabetes?


See your doctor to discuss management and review of your diabetes.

 

Are you between the age of 40-49 years old?


Come in and see one of our nurses and complete a ‘diabetes risk assessment tool’.
